What Formstack does

Formstack builds online forms, documents, and signature workflows without code. Teams use it for lead capture, registrations, HR paperwork, and internal approvals, often embedding forms directly on their own websites. Because submissions usually route onward into a CRM or a payment processor, an outage stops data arriving in systems well downstream of the form itself.

What an outage looks like

Embedded forms fail to render on the host page, leaving a blank area or a permanent loading state where the form should be, and visitors who do reach a form see an error on submit. The builder and dashboard can fail separately from form delivery, so authors are sometimes locked out while public forms still accept entries.

What to do about it

Check status.formstack.com, which resolves to the Intellistack status page following the company's rebrand. Pull embedded forms off high-traffic landing pages while they render blank, since an empty area converts worse than a plain link to a fallback. Before asking anyone to re-submit, confirm what actually arrived, because partial delivery is common in these incidents.

Why does status.formstack.com show Intellistack?
Formstack now operates under the Intellistack name and the status page redirects accordingly. It is the same service reporting the same incidents; only the branding changed. The old address still works, so monitoring pointed at status.formstack.com does not need updating.
My embedded form is blank. Is Formstack down?
Possibly, but check the embed first. A blank area appears both when form delivery fails and when the embed script is blocked by a content security policy, an ad blocker, or a recent change to the host page. Loading the form on its own hosted URL separates the two: if that works, the embed is the problem.
Do I lose submissions made during an outage?
Submissions that reached Formstack are held; the risk is the ones that never arrived. A visitor who saw an error on submit almost certainly did not have their entry recorded, and there is no queue on their side to retry from. Compare submission counts against traffic for the incident window before assuming everything landed.
Does a Formstack outage affect my CRM integration?
Only the flow of new submissions into it. Salesforce, HubSpot, or whatever receives the data stays up and keeps everything already delivered. Once Formstack recovers, check whether the integration backfilled the gap or whether submissions from the incident window need pushing through by hand.

How we measure this

  • We request Formstack's public endpoint every 6 hours from Fly.io regions across six continents — the most recent check ran from 8 of them.
  • A region counts as down only when it gets no usable HTTP response. A 403 or 429 means the origin answered and refused us, which we report as blocked, never as an outage.
  • A single failing region is treated as probe noise. We only change the verdict when two consecutive cycles agree.
  • Response times average only the regions that actually served the page, so a timeout never inflates the number.
  • Where Formstack publishes an official status feed we read it too. An all-clear from the vendor can soften an unconfirmed degradation; a vendor-declared outage only worsens our verdict when our own checks corroborate it.
